REACH proposes a collaborative initiative to enhance prevention and support services. <\/p>\n\n\n\n<p id=\"block-260babff-65c6-4b98-b935-5c75edc4fc8d\">Efforts include:<\/p>\n\n\n\n<ul class=\"wp-block-list\" id=\"block-63aa0374-2397-4aa1-98fb-1af13ba7c59f\"><li><strong>Community Education<\/strong><\/li><li><strong>Case Management and Support Services<\/strong><\/li><li><strong>Peer Mediation<\/strong><\/li><\/ul>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td><strong>Community Education<\/strong><br><br>Community education features violence prevention trainings and presentations for schools, community organizations, families,<br>and others.<br><br>To learn more about the trainings available and to schedule a training, contact YouthDevelopment@LUK.org or call 800-579-0000.<\/td><td class=\"has-text-align-left\" data-align=\"left\"><strong>Case Management and Support Services<\/strong><br><br>Case Managers will meet with youth weekly<br>wherever they are: school, afterschool program, community, home, and\/or virtually. Case managers will work with youth on goal setting that caters to the youth&#8217;s needs. Intervention and support services will include home visits, before and after school youth support groups, and simultaneous parent\/caregiver groups while youth participate in activity groups.<br><br>To make a referral, <a href=\"https:\/\/prevention.luk.org\/wp-content\/uploads\/sites\/3\/2022\/08\/LUK-Referral-Form-01.09.20.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">download our referral form<\/a> and return it to <a href=\"mailto:referrals@luk.org\">referrals@luk.org<\/a> or fax 978-829-2250<\/td><td class=\"has-text-align-left\" data-align=\"left\"><strong>Peer Mediation<\/strong><br><br>Peers Making Peace (PMP) is a peer mediation program designed to address student conflicts as they arise, preventing harmful conflict escalation.
